Jeffrey Milks, MD
Jeffrey Milks, MD has over 40 years experience in private practice and is board certified in Family Practice, Geriatrics, and Hospice and Palliative Care. A graduate of The Ohio State University Medical School, Dr. Milks is a member of the American Academy of Family Physicians, American Academy of Private Physicians, and the Ohio State Medical Association along with several other local professional associations. In addition, he has been awarded many honors, including OSU Department of Family Medicine Preceptor of the Year, OSU Teacher of the Year, Columbus CEO Lifetime Achievement Award and Columbus Business First Champion of Healthcare Legacy Award. He also prides himself on being the handyman of the office and has become an active pickleball player.

Dr. Brad Herpolsheimer
Dr. Herpolsheimer is board certified in family medicine and has been practicing for over 25 years in central Ohio. He graduated from Michigan State University in 1997 and then completed his residency at Grant Medical Center in Columbus, Ohio. His friends and colleagues call him “Dr. Brad”. Dr. Brad is a member of the American Academy of Family Physicians, was awarded the AOA-National Medical Honor Society while in medical school and was named one of central Ohio’s top providers in ThisWeek News’ 2016 Readers’ Choice Awards. He is an avid golfer and is a huge Michigan State fan.
James Wellbaum, MD
Dr. James Wellbaum is a board-certified family physician who has been providing primary care to patients in central Ohio for over a decade. He graduated from Ross University Medical School and completed his residency at Grant Medical Center in Columbus, where he served as chief resident. He is a member of the American Academy of Family Physicians. In his spare time, he enjoys spending time with his two young children, outdoor recreation and traveling.
Miranda Quintana, APRN-CNP
Miranda graduated from Ohio Northern University with a bachelor’s degree in biology and completed her master’s in nursing degree at The Ohio State University in June 2009. She joined Personal MD in September 2009. Miranda is available at our office to see patients for all types of visits, including geriatric assessments, Medicare annual wellness visits and women’s health exams. Our patients love her kind spirit and willingness to always go the extra mile. Miranda is an active mom to her three young children and enjoys reading in her spare time.
Deborah Milks, RN
Deb graduated from The Ohio State University with undergraduate and master’s degrees in nursing. She has been in the field of nursing for over 40 years, as a nurse, professor and nurse practitioner. She is one of the founders of the practice and enjoys developing relationships with patients throughout their lives. She enjoys spending time in Florida and with her grandson.
Lisa Gray, Clinical Manager and CMA
Lisa graduated from the Ohio Institute of Health Careers with her medical assistant certification in 2003. She completed her externship with OSU Family Practice, where she spent three years working as a medical assistant with Dr. Milks and Deb. She has been with the practice since the first day in 2006 and is adored by her patients. Lisa is the “green thumb” of the office and loves all types of animals.
Jill Wilson, MA
Jill has been a medical assistant for over 15 years. She has extensive experience in a variety of medical specialties, including gynecology, pain management, dietetics, primary care and nutrition. She enjoys teaching yoga, Pilates, cycling, and SilverSneakers classes in the Pickerington area. She joined the Personal MD team in 2023.
Holly Verhovec, Medical Receptionist
Hi, my name is Holly, and I am your Patient Care Coordinator here at PMD. I was born and raised in a small town in Wisconsin. My husband and I met while in the Army, bringing me to Utica, Ohio, where we have resided since. I finished my eight-year contract with the Ohio Army National Guard when I decided that healthcare is my passion. My medical background includes working in Emergency Medicine both as an EMT and an Emergency Room Technician for a few local Emergency Departments. I spend most of my free time with my husband and our two sons. We enjoy watching our sons play sports, especially baseball and going camping as a family. Sometimes you can find me snuggling my mini golden doodle, Phoebe while drinking iced coffee and reading a good book. It brings me so much joy to be able to work with my Personal MD Family. Getting to know all our wonderful patients is truly a gift!
